Provide a mechanism to customize the toc entries modularily

Currently, TocTreeBuilder is a internal utility class, with no extensibility mechanism.

Consequently, this is difficult to non-intrusively customize the entries of a table of content (e.g., adding numbers on entries from an extension - the use case of NCAPP).

This is leading to code duplication which is best avoided.
