Allow to inject a new column in default livetable columns

Currently the default columns of Livetables are hardcoded in LivetableMacroResults and it's not possible to inject a new one for all livetables of a wiki, in particular to modify at once all LT of Index pages.
It could be useful for some extensions to be able to perform such column injection, for example to display page ratings everywhere when ratings is installed.
