Unable to add into CB without an error (Image or existing video)

Create or edit a collection. Add an image using the add button in the CB. When you are done uploading and setting required info, you see an error "Image not available on server"- Happens on Current.dev.curriki.org

Also in the CB, if you add from site (I did a Video type asset), I see the same error, but the video does eventually display.
