Create Group Page - Issues

1) The group URL should be <blank>.groups.curriki.org instead of <blank>.curriki.org

2) Create groups form is not checking for any mandatory fields. I could create a group without even entering all the fields

3) Layout of form does not match treatment - No Hid show on title bars, vertical stacking, missing text elements like title for Subject and level, "I" balls, etc

4) Upon first creating a group, the interstitial "Click though" help message is not displayed as per the spec
