Roles - Adding Role Selection in Request and Join Forms

Add ability for users to select a role when requesting membership in a closed group. That role should be applied to the user when the Admin accepts the user on the Members page

Add ability for users to select a role when filling out the join form in an open group. That role should be applied to the users group profile.
