Add survey invitation into Review Alert Email

Consider adding a link to a survey directly into the CRS Review Alert Email rather than sending it as a separate process as is being done in summer/fall 2008. See notes in http://jira.xwiki.org/jira/browse/CURRIKI-2560.

Need to discuss with Karen (researcher), have survey updated, and determine what instruction text may need to be added. Also, may need to schedule periodic updates to the link to allow for collecting responses within a given period of time.
