Missing document in CRS document tab + missing resource

One of the CRS reviewers noticed these two missing assets:

http://www.curriki.org/xwiki/bin/view/Documentation_Group_CRSCollaboration/ReviewedResources-SocialStudies

Here are her notes describing the problem:

I was working on and off all day yesterday on reviews... going back and forth between the docs tab in our group and the repository to search for assets to review and/or update with a star rating. Yesterday, I was working exclusively on a Dell Vostro 1000 with Windows Vista. I was using Firefox. Usually, I work on a Macbook but it was done so I was using my husband's computer. I didn't have any technical problems until sometime after 9:00 pm when I tried to post my review of the Rome and Roman Britain collection and the error that I previously sent you came up. I tried it about three or four times and the same thing happened so I gave up and assumed I would try again in the morning. I then went to the docs page to write down the asset so I would remember that I still needed to upload the review. I opened up the Social Studies page and tried to edit it by writing "Rome and Roman Britain (6.30.09)." Usually I would then put in a link but since I didn't have one yet, I just tried to save and I got another error message similar to the one that happened with the review. I decided it wasn't my night and I shut down the computer. This morning when I tried to go back and upload the review, the asset was gone... although the pieces of the collection are there. Then I went to the docs page and found that it did not exist for Social Studies anymore...
