cannot request memebrship in a group on Production

On Production, I went to a group's Members tab:

I clicked the Request Membership button.

I completed all the fields on the Request Membership form and clicked the button to submit the form.

I got this error:

Error number 4001 in 4: Error while parsing velocity page Group_TAHSummerInstitute2009.RequestMembership Wrapped Exception: Failed to evaluate content with id Request Membership

Was able to reproduce 2x.
