add '?v=73645' to include file 'src=' for CSS and JS files

A simple enhancement that's good for production and even better for development/QA

The version number can be changed manually in a config file (the easy way?) or automatically by the build process - either way, when it changes, it forces all browsers to get fresh copies of these files instead of using copies cached by the browser.
