New feature and text for Corporate Donation Page

We need a way for the members to change their mind about their member type if they selected Corporate.

Add these lines to the corporate sponsorship dialog.

join.complete.corporate.membertype=Not a corporation? Select the contact link and we will reset your member type.
join.complete.corporate.membertype.link=Contact Support

When the member selects the link, we will reuse this form to automatically send an email to webmaster@curriki.org - the user does not need to fill it out:

In the email, include:
Username:
Email:
Note: Change the Member Type from Corporate to Teacher.

In the email subject line: Member Type Change

When the email is send, log the user out, close the sponsor dialog, display this dialog to the member:
