Logout and Change Member Type buttons missing in Corp Payment page

    Description

      1. Select Join Now link in Login dialog
      2. Fill out form correctly, selecting Corporate Member Type and Submit
      3. Validate using validation code
      4. The Membership Fee page appears

      Bug
      The dialog only has a Cancel button. It should have two buttons, one for Change Member Type and one for Logout and Pay Later
