Reviwer form must be completed, if not display error

Step 1 - Go to http://next.dev.curriki.org/xwiki/bin/view/CRS/Reviews?bc= to see a list of assets to review. If the list is empty, create one then nominate it then go back to the list.
Step 2 - Select an asset and select Review
Step 3 - Leave the Set ITC as Choose from list...
Step 4 - Select Submit Review

The Bug:

Reviewer must select an ITC, and all other choices for that matter.

2.4 Error Conditions

Reviewers may select to review resources that have recently been reviewed (but are still in their queue because they haven't refreshed) or 2 people are completing review form at once. This may not be an error in a technical sense?the last review entered will just become the current review. But we will instruct reviewers to note the 'last reviewed date' at the top of the review form, and if it is from the same day, do not review.

Review Form: All fields are required. Empty field --> message to prompt user: "Please complete all fields."
