Creating a question generates a duplicate when using MySQL

1. Install FAQ and create a new question: "a"
2. Create a new question: "A"
3. Check the FAQ entries.

Actual result:

• In the FAQ live table you will find 4 pages:
  2x "a?"
  2x"A?"
• If you delete one of the 4 pages, only 1 will remain in the live table.
• If you check the child pages of FAQ main page in the breadcrumb tree, there will not be any page duplicate.
