Feedback Form does not indicate "Title" as a required field

The "Title" field is mandatory and the form fails submission if this is not provided. This change will mark the field as required so users know that it must be entered.

Note that when the form submission fails, the user loses all of their information when the form resets (FEEDBACKFO-3).
