Add support for the pdftoc macro

Numbered heading does not appear when exporting to pdf.

This is caused by the use of the pdftoc macro instead of toc.

Note that with XRENDERING-704, this task should not be required and the corresponding code will be removed once NCAPP is upgraded to a version where XRENDERING-704 is included.
