Can't add evaluation on any browser IE 11, Chrome 57, Firefox 52 after I've Saved & View 2 times or Save & View then add a Cover Letter or/and a CV

Steps to reproduce:

1. Log in as Admin
2. Add a new Entry
3. Fill in the information required in Personal Information
4. Click Save & View
5. Click Edit
6. Change some data in Personal Information
7. Click Save & View
8. Click Edit
9. Go to the Evaluation tab

ALSO you get the same issue with the following steps:

1. Add a new Entry
2. Fill in the information required in Personal Information
3. Click Save & View
4. Click Edit
5. Add info and a Cover Letter or/and a CV
6. Go to the Evaluation tab

Expected results: Can add information about the Evaluation - grading, description.

Actual results: Can't add anything in the Evaluation tab.

Note: It appears that after 2 Save & View or after 1 Save and View and adding an attachment that makes the page refresh the Evaluations tab doesn't work anymore.
