Not able to add new pages in XWiki Debian 15.4

Hi,

I am finding that in 15.4, if I try and add a page for review, as soon as I select the page I am redirected to a page called 'undefined' at ReviewPages.undefined.WebHome

I never get the chance to add the date of review.  I have tried uninstalling and re-installing the extension. 

Any help with this would be much appreciated - its just what we need!
