Implement Cas Consumer handling for pure UIMA environment.

After the big scribo-api refactoring, the cas consumer always used a temporary knowledge base.

It needs to be fixed so that, in a pure UIMA environment, it gets the knowledge base parameters from the UIMA context and uses the actual knowledge base instead of a temporary one.
