Add "Parent" and "Title" fields to Default Class Sheet

In the "Create a new document" section of the Default Class Sheet, there are only "Space" and "Document" fields. I think there should be fields for "Parent" and "Title" as well so the page creator can set these before page creation rather than after.

The addition of a "Title" field would be more consistent with the new convention of displaying the document title as a header at the top of the document. As it stands in the default Default Class Sheet, no title is set and therefore the first header is displayed as the title at the top of the document. This is a source of confusion as one would expect the title to at least be set to the name of the page instead of being left blank.
