TOC macro with local scope can include non local headings

To reproduce:

• Create a Main.TOC.WebHome page with the following content

  = hA =
  
  {{toc start=2 scope=local/}}
  
  == hAA
  
  == hAB
  
  {{include reference="Main.TOC2.WebHome"/}}

• Create a Main.TOC2.WebHome page with the following content:

  = hB =
  
  == hBA
  
  == hBB

Then view Main.TOC.WebHome and you'll see what's attached.
