Table of Contents Macro: Handle empty headings

Would be nice if the Toc macro was more explicit about missing heading text. This would encourage users to correctly format heading hierarchies.

If a line is marked as a heading but doesn't contain text then the Toc macro could display something like "<Missing heading text>" instead of a blank bullet point.
