Log the number of appearances for each macro on each page

Currently, a list of all the macros found is provided at the end of the filter stream conversion. It would be more useful to log the list of macros for each page together with number of occurrences. This way, for each page we would know the macros used and how many times but also know all the pages where one given macro is present.
