Only add a glossary link for the first occurence of a glossary term

It happens often that a page will contain several times a glossary word and it makes the page quite heavy to read when there are lots of links in it, so the idea of this issue is to introduce a configuration option to only link the first occurence for each glossary entry, when using the transformation or the "update page on save" strategies.
