Disconnected instances are not taken into account properly

    Description

      It's possible that in some cases the instance referred to by some followers is not available anymore for many reason.
      We should handle properly those cases. Right now we had the following problems related to that:

      • notifications broken because of mentions of external users
      • sending of activities (messages / creation of page) broken because of error when sending it to followers
