Make the "Template name" field mandatory

Currently the "Template name" field isn't mandatory. Because of this an user can create multiple templates without a name and forget which one is which (check the attached screenshot).

Steps to reproduce:

1. Hover over Wiki and click on "Administer Wiki"
2. Click on Templates
3. Write a "Template Provider" name in the "Page" field
4. Click on "Create"
5. Write a name in the "Template name" field
6. Write a name in the "Template to use" field

I propose making the 5th step mandatory.
