The annotated HTML5 renderer doesn't render all metadata for macros

Compared to the annotated XHTML 1.0 renderer, the annotated HTML5 renderer misses the rendering of metadata blocks (beginMetadata/endMetadata, see XRENDERING-527) and also doesn't render macro markers for nested macros.
