Capitalize the names of default fields on AWM

Steps to reproduce:

1. Go to "More Applications" from "Application" panel
2. Click on "Create your own"
3. Start creating an app
4. Add some fields
5. Click on "Next" button
6. You see the "Step 3"
7. Do not add your new fields on, just let the defaults ("Page name", "Update date", "Last Author", "Date") and add some generic ones
8. After you save the app, go to the app homepage
9. The fields are not all capitalized
