Description
The goal of htmlHeaderAndFooter is just to provide technical <html>, <head>, <body> around the document content. Ending up with things like the menu is an important bug that make this option totally useless as soon as you install something like the menu application.
For example compare the result you get with http://www.xwiki.org/xwiki/bin/view/Main/?xpage=plain and http://www.xwiki.org/xwiki/bin/view/Main/?xpage=plain&htmlHeaderAndFooter=true.
