Unable to add more than 1 comment when CAPTCHA is enabled

After adding one comment, while CAPTCHA is enabled, regardless if the add is successful or not (i.e. CAPTCHA answer is correct or not), you encounter 2 problems:

1. If an Image CAPTCHA is used, with JCaptcha, the image will be the same every time (until you refresh the page)
2. Once you submit the new comment form once, after the docextra comments section is reloaded, the JavaScript elements of the CAPTCHA stop working. When using an Image JCaptcha, you will notice that the Refresh button is not working and, when using Sound JCaptcha, you will notice that the Listen button is not working either. It's even more problematic when using reCAPTCHA, since it will fail to load completely (after the submit).
