Duplicate versions in history of documents not based on JRCS

To reproduce the issue, import the attached package or any other input filter that generates the history document by document.

The resulting history of a page will have the following issues:

• The first version of the document will be duplicated
• The current version of the page will be the second last
• The last version will be corrupted - viewing it will return 404
• The second last version will have the content corresponding to the last version but the correct version comment
