Introduce a macro to create a page from a template

Currently, there are two ways to create a page from a template:

• Using the list of templates proposed when creating a new page
• Using a crafted URL with a template parameter (e.g., AWM's add entry)
  But, an advanced user cannot create easily a form allowing less knowledgeable users to create pages with a pre-determined page template

Macro parameters

• Default space
• Template
• is the iuser allowed to change the space

Result
The macro renders a form with the following fields:

• A page title field
• A "create page"

Note
Could be a replacement for the creable blog page form for instance
