The selection checkboxes of the Extension History have no label

Steps to reproduce

1. Install an extension.
2. Go to Administration > Extensions > Extension History.

Expected

Each record checkbox has an accessible name.

Actual

The <input type="checkbox" name="extensionHistoryRecord"> elements have no label, aria-label or aria-labelledby. They are rendered by the displayExtensionHistoryRecord macro of templates/extensionHistory.vm.

Context

Found by the WCAG validation of ExtensionHistoryIT#testInstall, which reports the rule "Form elements must have labels" on 6 checkboxes.
