Consistent buttons and titles in dialogs

Here is a list of suggestions that could help improve consistency in dialogs:

The title for image insertion is "Insert or edit image". As a result I suggest:

• changing the Table dialog title from "Table" to "Insert Table"
• changing the Macro dialog title from "Macro Insertion" to "Insert Macro"

The button for image insertion is "Insert Image"; the button for macro insertion is "Insert Macro".
In order to insert a table there is an "Insert" button. I suggest changing it to "Insert Table".
