PDF Export doesn't list the Table of Contents under certain circumstances

    Description

      Steps to reproduce:

      1. Navigate to a document which has at least a couple of headers (for ex. go to Sandbox.WebHome)
      2. Hover over Export and click on "Export as PDF"
      3. Select the "Table of Contents" and the "Image Attachments" checkboxes
      4. Click on Export

      The resulting PDF lists a blank page with the "Table of Contents" title. This bug can also be reproduced if there is at least one comment for that document and instead of the "Image Attachments" checkbox you select "Comments" in the 3rd step.
