Expand Lightbox scope to also include images in the document title area

Currently the Lightbox application operates on any image contained in the #xcontent area. This issue is about expanding its scope to the .xcontent area which is wider and includes the document header. In some cases indeed, the document header iself can contain one or several images, typically when using a cover image before or under the title.
